New Patient Instructions Prior to First Visit

1. Complete and sign the new patient forms.

2. Please read and sign the patient release form for the mercury vapor test.

3. If you are interested in having the Mercury Vapor Test performed, please DO NOT eat or chew anything, drink anything hot, or brush your teeth for at least 90 minutes before your appointment. (The heat or any friction will cause toxic mercury vapors to plume out of your "silver[-mercury] fillings". Your base-line will be low when compared to your stimulated readings. We will give you gum to chew, and our mercury vapor analyzer will measure your levels pre and post.) Then we will chart your results and explain the meaning of graph to you. 

4. Be sure to bring the filled-out new patient forms with you on the day of your appointment (plus any prior X-rays, test results, or reports you wish me to consider when evaluating your situation). We will then take the necessary information and records, to come up with a comprehensive treatment plan that we can discuss with you and complete for you as you wish.
